Our mission is to enhance the lives of our patients. The goal: always to go above and beyond all of the other therapy clinics and to make a difference, not only in the lives of the patients we treat, but also for the families of those patients. For this reason, we named our company Above & Beyond Physical Therapy, thus perfectly reflecting the philosophy of the founding partners.
Above & Beyond Physical therapy provides physical therapy to patients throughout the valley with clinics located in Phoenix and Queen Creek. Our team of professional, licensed physical therapists also believe in our philosophy and goal and treat all age groups.
Our team of experienced physical therapists are dedicated to recovering all patients to optimal performance and function. We are committed to treating our patients as family, and ensuring a positive experience for every individual who comes through our doors.
TOM LONG, PT | PHOENIX & QUEEN CREEK
Tom is a native of Illinois, and grew up in the small suburb of Naperville, Chicago. He attended Midwestern University in Downers, Illinois and displayed his leadership abilities as President of his physical therapy class and of the College of Health Sciences. Tom also received the Midwestern Community Service Award while completing his degree. Tom has extensive training in spine rehabilitation, including lumbar, thoracic and cervical disk issues, pelvic girdle asymmetries, radicular symptoms and cervico-genic headaches. He enjoys treating patients with spine and sports injuries, and with his experience as a physical therapist and athlete, can relate to his patients wanting to return to the activities they love.
Tom was able to realize his dream of opening a physical therapy clinic in 2000, and provides physical therapy in a fun, warm inviting, and family-oriented atmosphere, while achieving outstanding patient results. He is committed to quality patient care, but also likes mixing things up a bit by telling silly jokes and having fun. Living by his company motto, “Your Recovery is Our Passion”, Tom is very passionate about physical therapy, patient care and family. hether it is his Above & Beyond staff or his wife and son at home, both families are very special to him. Tom enjoys spending time with Carrie, his wife, and their son and daughter. He also stays busy playing soccer, skiing, golfing, hiking and competing in triathlons.
TED GIFFORD, PT
Charles goes by his middle name, Theodore, or Ted. Ted completed his physical therapy training at the University of Southern California. He practiced in southern California until 1999, when he moved to Phoenix. Ted began working part time for Above & Beyond Physical Therapy in 2009. Ted has completed post-graduate training in joint and spine manipulative therapy and orthopedic physical therapy. Ted enjoys a good work out at the gym and spending time with his horse Cochino.

CARL BISCONTINI, PT, DPT, CSCS, TPI-CGFI, ACSM-HFS
Carl is originally from North-Eastern Pennsylvania. There he obtained his Bachelor’s Degree in Exercise Physiology with a Concentration on Strength and Conditioning from East Stroudsburg University, and received his Doctorate of Physical Therapy from Temple University. Carl also holds certifications from the Titleist Performance Institute as a Certified Golf Fitness Instructor and a Certified Functional Movement Screen Administrator. He is currently continuing his education in Postural Respiration and Myokinematic Restoration with the Postural Restoration Institute.
Whether it’s working with pediatrics or professional athletes (Major League baseball, National Football League, National Basketball Association, PGA Tour, Web.com Tour), Carl enjoys helping patients by integrating corrective functional strategies with a hands-on approach to treat common movement dysfunctions and return them to a healthy, pain-free lifestyle.
Carl resides in Gilbert, enjoys playing golf, watching any sporting event, and spending time with his family and dog, Humphrey Bogart.
LEVI STREET
Levi lives by the mantra “medicine adds days to life, physical therapy adds life to days.” He graduated from ASU in 2011, and then moved on to A.T. Still University, from which he graduated in 2016. Levi is trained in spinal manipulation, which is similar to a chiropractor’s adjustment and can help restore motion to the spine and reduce pain. He is also a Matter of Balance coach at his alma mater ATSU’s fall prevention program which has been shown to decrease fall risk and the fear of falling.
When Levi isn’t working he enjoys spending time with his family and going for a nice motorcycle ride.
